Venu du centre de l'Angleterre, ce groupe a été le premier à associer hard rock et magie noire invoquant les forces du mal sur fond de rythmiques lourdes et d'atmosphères pesantes, dans ses tubes "paranoïd" et "iron man". "Sans Black Sabbath, a dit un jour Lars Ulrich à un journaliste de Mojo, il n'y aurait jamais eu de Metallica. Kurt Cobain a décrit le son de Nirvana à ses débuts comme un croisement entre les Beatles et Black Sabbath". Outre une influence majeure sur tous les groupes de hard rock et ceux affilés au mouvement Death metal, il a inspiré des formations aussi diverses que Guns and Roses, Biohazard, White Zombie, Megasdeth, Theraoy, Pantera, Skunk Anensie, Ministry, Faith no More (qui a repris "War pigs" en 1989" et le rappeur Ice T.
